For the most commonly used basic dental services, simply pay the low per-visit copay and any other applicable copays ($20/$30 based on the service provided). Major services are provided beginning on the first day of the seventh month of continuous coverage under this contract and paid at 20% of the provider's usual charge for that service (plus the per-visit charge).
